Your pet must have a yearly exam done with Clinton Veterinary Hospital. 

Canine Vaccines:

• Rabies
• DA2PP (Distemper)
• Bordetella
• Influenza

Canine Annual Testing:

• Heartworm, Lyme, Ehrilichia, Anaplasma
• Intestinal Parasite Panel

Canine Prevention:

•  Heartworm prevention 
• Flea & Tick prevention

(purchased from Clinton Veterinary Hospital or proof of purchase from another hospital, ie. receipt/medical record)

Feline Vaccines:

• Rabies
• FVRCP (Distemper)

Feline Annual Testing:

• FIV, Leukemia, Heartworm
• Intestinal Parasite Panel

Feline Prevention:

• Flea & Tick prevention
